Interprofessional Team Conference​
 
During my externship placement at Winchester Rehabilitation Center, I participated in weekly team conference meetings. Participating members of each team conference included the patient's medical doctor, nurse, physical therapist, occupational therapist, speech-language pathologist, neuropsychologist, and social worker. Each week during team conference, I reported on the patient's goals, progress, and discharge recommendations. I also listened to reports from the other team members to make adjustments to my patient's plan of care based on their findings, if warranted.
